Q: I have 1 doctor saying he uses drains on everybody and the other doctor saying he never uses them! Deciding between the 2 and I just don’t know! Both are board certified and do the procedure often

A: The use or not of drains shouldn’t be the deciding factor when choosing a plastic surgeon. Look at each surgeon’s results and reviews. I use drains regularly. I am aggressive with liposuction. This can lead to fluid accumulation in the treated areas. These fluid collections are a major source of post-operative headaches and leaving them in place can cause lumpiness. While drains are admittedly inconvenient, they are only temporary, they decrease seromas and infection and, I believe, lead to better long term results. Every surgeon is different and some feel they have great results without drains. I think the results are more important rather than this small detail in the process.

Q: I am curious if I can get good results with a mini TT. I’m 26, haven’t had children but lost around 70 lbs 10 years ago. I don’t think I have muscle laxity and would rather not have them tightened.

A: A mini tummy tuck only addresses the area below your belly button. This is only a good option for people with no skin laxity in the upper abdomen. Based on your pictures, I don’t think that would be the best option for you. I have many patients like you who need a tummy tuck, but don’t want or need muscle repair. I offer a modification of the full tummy tuck (I call it a J tuck) that does not disturb the abdominal muscles but still tightens the entire abdomen.

Am I able to have a Tummy tuck?

Q: I just turn 21 and I had my baby girl 9 months ago. I have really bad stretch marks and loose skin. I just want to know if I could be a candidate to do the tummy tuck.

A: A tummy tuck is a great surgery to reverse some of the unpleasant consequences of pregnancy. It can remove loose skin – this will also decrease the number of stretch marks you have on your abdomen. During a tummy tuck, I am also able to repair abdominal muscles that have been stretched out during pregnancy. 9 months is generally enough time after childbirth for you to have fully healed. Ideally you should be at your goal weight before pursuing plastic surgery and I generally recommend waiting until your family is complete before having a tummy tuck. Major changes – like large weight loss or another pregnancy – can undo the effects of the surgery.

If you have stretch marks does it automatically mean you need a Tummy tuck , after lipo?
The presence of stretch marks does not automatically mean you need a tummy tuck. A tummy tuck does two things – it tightens the underlying abdominal muscles and it removes excess skin. The danger when performing liposuction is that removing fat from an area could leave the skin in the treated area loose. Skin with stretch marks does have less elasticity than areas that don’t have stretchmarks. Because you are planning a family in the near future you have two options. The first is just to proceed with the liposuction and see what happens. There is absolutely no reason why you cannot have a tummy tuck in the future if you need one. If you and your plastic surgeon are concerned that your skin may not “snap back” after liposuction, you could consider what I call a J tuck – a skin only tummy tuck.
