Q: My BMI is 31 and have been stuck here for five years even with tons of cardio, eating sensibly & lifting weights. At age 37 I have a healthy resting heart rate of 58-61bps but still overweight at 30% body fat. At my heaviest years ago I was 210lbs (after having baby) now 170lbs and stuck here way too long 🙁 Would just lipo help me or do I need a tummy tuck as well to get flat stomach? What do you think about combining above with fat transfer to hips/butt to achieve illusion of smaller waste?

A: I think you will get your best result from a Tummy Tuck with Brazilian Butt lift and possibly some additionalliposuction. Performing liposuction alone on your abdomen will not address the underlying muscle looseness and will probably leave you with loose hanging skin. Pregnancy stretches the abdominal muscles. Unfortunately exercise and diet alone will not repair them – as you have seen. You have been very disciplined but have probably done as much as is possible without surgery. I perform these procedures together for healthy women in my surgery center outside Atlanta. I would suggest your next step would be a consultation with a board certified plastic surgeon.

Q: My husband was to be home for a 8wks but just got orders for school 5 days after my sx. My mother-in-law is going to be here the first 7 days. How much help I will need since my husband is military and can’t change orders for school. Mind you we just found out today, sx is in 7 days. I have a 22m, 8 yr, 6yr. My older kids will be able to help alot with the youngest but they have school. How much help do I need to have during the day by day 7, is it safe for me to be home alone with the baby?

A: I would definitely recommend your having adult assistance at home for at least 2 weeks following surgery. You should not lift more than 10 pounds for at least that time. You need to protect your results by following your doctor’s instructions about rest and positioning. While your older kids can understand and can probably do a lot for themselves, there is no way to safely supervise a toddler alone during those first 2 precious weeks.

Q: I am on a workout regime but this area is giving me a hard time. I ‘m considering liposuction on my upper back (around my shoulder blades). Would that reduce my back fat to allow the zipper to close without appearing too tight?

A: Liposuction of the back bra roll and upper back is challenging, but possible. The most challenging part is managing expectations. While patients and surgeons both want the rolls in this area completely gone, this is typically not possible. Generally, you can expect a 60% improvement in this area, but most likely you will still have some residual roll – especially if you have excess skin in this area. Fat in this area is typically denser and more fibrous than fat in other areas and the back skin is firmly stuck to the tissues underneath. The other tricky issue with liposuction of the upper back is scar placement. I typically do not use incisions directly on the upper back because they would be visible when you wear backless tops, swimwear, etc. Patients with heavy back bra rolls with redundant skin may require excision of these rolls – generally this puts a visible scar in the middle of the back near the bra line. These are some of the options for you and your plastic surgeon to discuss.
