A: There are several issues to discuss in your question. First, I frequently combine Liposculpture, Brazilian Butt lift and breast augmentation in healthy patients. It does complicate the recovery process when I operate on the front and back of the body at the same time. You will need to lay on your sides for several weeks after surgery.

It is impossible to tell whether or not you can achieve your desired abdominal results with liposuction alone without seeing you. I can tell you that liposuction does not create elasticity of the skin. If you have stretchmarks and loose skin on your abdomen liposuction alone will not fix that. In fact, if you already have loose skin after pregnancy, liposuction without a tummy tuck may make the appearance of loose skin even worse. You will need a consultation to determine whether or not you would benefit from a tummy tuck.

I am VERY concerned about surgery performed outside the US. While there are certainly qualified doctors in other countries, I would recommend your staying in the US for your surgery. Different countries have different standards for education and licensing potentially making it hard to determine who is qualified to perform your surgery. Language barrier could be an issue as well as access to follow up care. It will be VERY hard to convince a US plastic surgeon to take your case if you have a complication after surgery outside the country.

A: Having liposuction now will not affect a future pregnancy. It will not, however, prevent pregnancy related body changes. Having liposuction now doesn’t mean you might not want a tummy tuck after you are finished having kids.

A: You sound like an ideal candidate for a tummy tuck. Based on your pictures, you have stretchmarks around your navel. Stretchmarks indicate to me that your skin has been damaged by excessive expansion – in your case it sounds like this occurred during your pregnancies. This damaged skin is never going to snap completely back – it doesn’t matter how much you exercise, watch your diet or apply creams. If you want it gone, it will need to be surgically removed. Scarring is an important consideration – an abdominoplasty does create a scar low on the abdomen from hip to hip and often a scar around the navel – but remember you are trading contour for scar. Also, in the hands of a skilled board-certified plastic surgeon, the incision is hidden by most underwear and swimwear.

A: There are certainly fantastic plastic surgeons around the world, but outside the US, it can be difficult to be sure that you have selected one. Many patients seek plastic surgery outside the US due to the lower cost, but that can be a risky proposition. Different countries have different medical education and licensing procedures. Traveling outside the US can increase your risk of acquiring an infection. There is also a potential language barrier and you need to fully consider access to follow up care. By selecting a physician inside the US, you have many methods to research your doctor’s education, training and qualifications for the procedures you are considering. Hopefully you won’t experience a complication, but if you do, it is much easier to obtain the necessary follow up and treatment when you have surgery domestically.

A: We have no evidence that fat transfer to the breast causes breast cancer or in anyway changes your cancer risk. It is possible that the surgery could produce benign calcifications that could be confused with breast cancer on a mammogram. This could lead to your needing further investigation or even a biopsy.
