Q: Do I absolutely need the periareloa lift my ps is telling me I need. He plans on cutting out th areola. Repositioning it. & I will have a vertical scar from my nipple to under my breast & then a horizontal line under the breast as well. Is there Any other option to avoid all these scars? I want bigger full fake looking breasts. Thanks!

A: In the picture you submitted, you have your arms raised to hold the camera and your nipples are below the breast crease (inframammary fold). I would most certainly recommend a lift. Without one you would need your implants placed over the muscle which would contribute to breast drooping and not give you any upper fullness. With your degree of breast sagging any other implant placement (either dual plane or under the muscle) would likely give you a double bubble. Many of my breast augmentation patients want to create lift with implants alone, but this simply doesn’t happen. A breast lift will allow your surgeon to lift your breast tissue upward on your chest and place your nipples in a perky position. Only you can decide if the resulting scar is worth the improvement in shape.

A: Women can ABSOLUTELY have abdominal etching, also known as high definition liposuction! In my surgery center outside Atlanta, I use VASER – ultrasound assisted liposuction – to remove fat from the abdomen and highlight the abdominal muscles. The result is a sculpted, feminine abdomen. Vaser has the added benefit of tightening the skin so the results are truly amazing.

A: You should play it by ear. I generally recommend allowing 2 weeks for recovery after liposuction. Everyone is a little different. My rule is – if it hurts, don’t do it.

Q: Is it only the fat removed or does it include the injected fluid? For example, if 5 liters was removed, is that all fat or is part of that the added fluid? Do doctors calculate this differently? I’ve never really understood this part. Thanks so much!

A: During liposuction, tumescent fluid – sterile saline mixed with medications to control bleeding and pain – is injected into the treatment area and allowed to sit for a time. This tumescent fluid is critical for successful liposuction. It loosens the fat and reduces complications. Tumescent fluid is then removed along with the fat. I count the total volume of fluid and tissue removed and never exceed 5 liters in one procedure. The liposuction aspirate contains fat, tumescent fluid and blood. Many patients just want to count the amount of pure fat removed, but unfortunately this is impossible to do. First, there are microscopic amounts of fat mixed in with the fluid. Also, the risks of liposuction increase when more than 5 liters of aspirate is removed – even though that aspirate contains more than just fat.

What a great question! I tell my patients that breast implants are like tires and will eventually need to be replaced. I expect breast implants to last about 10 years, so yours have lasted longer than expected. Since you are already undergoing breast surgery, I would recommend replacing the implants during the procedure.

A: The best answer is to ask your plastic surgeon. We all have different recommendations, plus your surgeon can tailor his or her advice to your specific case. I typically perform large volume liposuction on multiple body parts. Often, I am performing other procedures, like fat transfer to the hips and butt or a tummy tuck as well. I generally recommend my patients give themselves 4-6 weeks before exercise.
