Q: I have lost a huge amount of weight during the past few years. I am looking to get a tummy tuck and a breast lift within the next 6-8 months but I have a question. I have stretch marks(all white/old ones, not red ones) all down my stomach and hips, and also have a lot of stretch marks UNDER my breasts. Will a tummy tuck and breast lift get rid of any stretch marks? If so, about how much will be gone? If not, is there anything at all that would help get rid of them?

A: Wonderful question. Skin is removed during both a tummy tuck and breast lift. If that skin has stretchmarks on it, those stretch marks will be removed. Stretchmarks in other areas won’t be removed, but will likely change position. Your plastic surgeon should be able to give you a good idea of which will move and what your skin will look like after surgery.

During a full tummy tuck, the entire abdomen is addressed from the rib cage to the pubic bone. During a full abdominoplasty I loosen the tummy skin all the way up to the chest. The abdominal muscles are tightened and loose skin is removed. I generally save the existing belly button. Most of my patients who need a full tummy tuck desire liposuction as well and I usually combine these procedures. It is not uncommon for many of my tummy tuck patients to add on additional procedures like liposuction elsewhere, breast procedures or a Brazilian Butt Lift, as well!

Q: How much weight (in loose skin/tissue) will I lose from a tummy tuck?

A: There are many variations on the tummy tuck, but it generally involves tightening the abdominal muscles and removing excess skin from the abdomen. The skin and tissue removed usually weigh a couple of pounds, but without seeing you, I can’t make an estimate that is specific for you. I encourage my patients – especially my tummy tuck patients – to achieve and maintain their goal weight PRIOR to surgery. Tummy tucks don’t help with weight loss and losing a significant amount of weight after surgery could lead to loose skin again.

A: The use or not of drains shouldn’t be the deciding factor when choosing a plastic surgeon. Look at each surgeon’s results and reviews. I use drains regularly. I am aggressive with liposuction. This can lead to fluid accumulation in the treated areas. These fluid collections are a major source of post-operative headaches and leaving them in place can cause lumpiness. While drains are admittedly inconvenient, they are only temporary, they decrease seromas and infection and, I believe, lead to better long term results. Every surgeon is different and some feel they have great results without drains. I think the results are more important rather than this small detail in the process.

Q: I am curious if I can get good results with a mini TT. I’m 26, haven’t had children but lost around 70 lbs 10 years ago. I don’t think I have muscle laxity and would rather not have them tightened.

A: A mini tummy tuck only addresses the area below your belly button. This is only a good option for people with no skin laxity in the upper abdomen. Based on your pictures, I don’t think that would be the best option for you. I have many patients like you who need a tummy tuck, but don’t want or need muscle repair. I offer a modification of the full tummy tuck (I call it a J tuck) that does not disturb the abdominal muscles but still tightens the entire abdomen.
